  1. For a health insured person Certificate of entitlement № 7 issued by the family doctor or a specialist which is valid a month after issuing;
  2. Laboratory tests – only tests within 7 days from the admission are valid
  3. Documentation of preliminary consultation with other specialists in case of comorbidities
  4. ID card
  5. Thermometer
  6. Toiletries
  7. A Nightgown, slippers, bathrobe and cutlery
  8. Sanitary towels, wet wipes
  9. Medical documentation from previous hospital stay or examinations, Personal outpatient card, stamped and signed by the family doctor
  • Blood tests to patients who will have gynecologic surgery can be performed in office of 219 fl. 2, after being provided with the required slip from the relevant office (207 and 214).
  • Before admission to the clinic the patient has to go through hospital registry check.
  • Every newly admitted patient leaves her clothes at the hospital’s locker room which is located in the main lobby of the hospital.

The issuing of work excuse shall be made on the day of discharge from the hospital by the senior midwife between 9:00 and 12:00 hrs. Medical history is obtained on the day of discharge from the hospital, if there is ready histological result, and if not – from the registry – fl.2 after consulting on the phone – 02/9172 442.

 

 

  • The patient could not keep medical records or a part thereof. The documentation should be kept in the manipulation room of the clinic with the midwife.
  • References and additional information can be obtained from 13:00 to 14:00h – tel. 02/9172286 or 02/9172 223 – Gynecology office or Oncologic prevention office (2nd floor office 207 or office 209)
